Fees:
If you'd like to pay out-of-pocket, I charge a customary fee of 150.00 per 55-minute session and 200.00 per 90-minute session Please note that hypnotherapy and extended EMDR sessions are for regularly scheduled clients only--at this time I am not offering brief therapy sessions. If you plan to pay privately, please read the Right to Receive a Good Faith Estimate form listed below.
Two-hour PLSR sessions are 245.00. Please note that sessions longer than 55 minutes are generally not covered by insurance, and PLSR is never covered by insurance. Due to the COVID-19 pandemic, sessions are now also offered on a HIPAA- compliant platform. In-person sessions are offered to clients who are fully vaccinated and boosted.

Please bring in or send your insurance card for me to copy, and please also be informed about your insurance, such as: your co-pay/co-insurance, your deductible, and whether you have a Health Savings Account (HSA) or Health Reimbursement Account (HRA). You can find this information by calling the number for customer service on the back of your card.
Cancellation Policy
Sessions not cancelled or rescheduled within 24 hours (weekdays only) will be charged a 50.00 fee. PLSR sessions require 48 hours notice for cancellation or rescheduling.
Forms:
Please either print and fill out all required forms, or plan to arrive about 15/20 minutes early to your first session to fill them out at my office.
If you are an adult client, please print, fill out and bring in: Client Rights and Disclosure form, Consent to Treatment form, Notice of Privacy Practices, Adult Client Information form, Social Media and Telecommunication form and Teletherapy Consent Form (if applicable).
If you would like therapy for your child, please print, fill out, and bring in: Client Rights and Disclosure form, Child Client Information form, Consent to Treat a Minor form, Notice of Privacy Practices form, Social Media and Telecommunication form and Teletherapy Consent Form (if applicable).
For divorced or separated parents:
Divorcing and separating from your partner can be highly stressful for both of you as well as for your child(ren), however research shows that children can adjust in some wonderful ways as long as both parents are willing to work together and truly co-parent. When a child sees their divorced parents communicating civilly, speaking in a neutral or positive way about the other parents and encouraging a positive and warm relationship with that other parent, that child can be emotionally resilient. However, research also shows that children suffer significantly when parents fight in front of the child, refuse to communicate or speak ill of the other parent. Progress in therapy can be slow as the child continually manages their emotions of being stuck in the middle, feeling torn in their loyalties.
As it is in the best interest of your child, I am only able to work with divorced parents who are willing to co-parent civilly. If you are struggling with a highly conflictive relationship or are battling for custody and are not willing to co-parent effectively, I am unfortunately not able to see your child. Since I would be solely your child's play therapist, I am also not able to engage in the dual relationship of supporting you legally (including working with CFI's and attorneys and testifying for you in court).
Please note that if you are divorced and share custody, it is legally required that both parents consent to treatment (and sign the Consent form). In addition to the above forms, both parents also please print and sign the Agreement for Divorced and Separated Parents. On your first visit, please also bring in a copy of your divorce/custody agreement.
